Athletic Training - Forms

Prior to practicing or competing in any NCAA allowable event/practice for the University of Minnesota Duluth, all student-athletes need to complete the medical eligibility process. 

 

All INCOMING and TRANSFER student-athletes to UMD need to complete the following forms in hard copy format and return to UMD Sports Medicine:

  • Student-Athlete/Insurance form  

  • Physical form

  • A current copy of their insurance card (front and back)

ALL student-athletes must then complete the remainder of paperwork online and will receive an email from NExTT Solutions (the online database which houses all medical information and is encrypted with security measures).  The forms included are:

  • Assumption of Risk/Consent for Treatment form

  • Medical History form

  • Sickle Cell Testing Waiver form (online if waiving testing, otherwise testing needs to be completed prior to arriving at UMD)

Finally, as an athlete of an NCAA accredited institution, you may be selected for randomized drug tests. It is imperative to both your athletic eligibility, as well as your health, that you not ingest any of the substances found on the NCAA Banned Substances list.  The following link will direct you to the NCAA Banned Substance list for your reference:
